中国抑郁障碍人群终身患病率高达6.9%，其中现症患者人数超过9 500万例，其病程迁延，复发率高的特点不仅给患者及家属带来巨大身心痛苦，也造成沉重的社会负担。非药物治疗是当前临床抑郁障碍全病程综合干预过程中的重要组成部分，针刺治疗抑郁障碍凭借其疗效显著、成本低以及不良反应小的独特优势，已成为未来抑郁治疗领域的重要研究方向。抑郁障碍的发生与神经反射、神经环路异常密切相关，基于“从脾治郁”指导下的针刺治疗可以弥补单一从肝论治的局限性，更贴合慢性、反复发作型抑郁障碍患者虚实夹杂的病理特性，针刺刺激与脾相关的穴位，已经被证实可通过激活体表神经末梢，触发相关神经反射，影响中枢神经环路，缓解抑郁症状。因此，本文对近年来“从脾治郁”理论指导下针刺治疗抑郁障碍的神经调控机制进行综述，以期为未来研究提供借鉴与参考。

The lifetime prevalence of depressive disorders in China reaches as high as 6.9%, with over 95 million current patients. The chronic course and high recurrence rate of this condition not only impose significant physical and psychological burdens on patients and their families but also create a substantial social burden. Non-pharmacological treatment constitutes a crucial component of comprehensive clinical intervention throughout the entire course of depressive disorders. Acupuncture therapy for depressive disorders, with its distinct advantages of remarkable efficacy, low cost, and minimal adverse effects, has become an important research direction in future depression treatment. The onset of depressive disorders is closely associated with abnormalities in neural reflexes and neural circuits. Acupuncture therapy guided by the principle of "treating depression from the spleen" addresses the limitations of single-focused treatment approaches targeting the liver, better aligning with the pathological characteristics of chronic, recurrent depressive disorders characterized by a mixture of deficiency and excess symptoms. Stimulation of acupoints related to the spleen has been proven to activate peripheral nerve endings, trigger relevant neural reflexes, influence central neural circuits, and alleviate depressive symptoms. Therefore, this article reviews the neuroregulatory mechanisms underlying acupuncture therapy for depressive disorders under the guidance of the “treating depression from the spleen” theory in recent years, aiming to provide insights and references for future research.
